The proper thing for case D is what you describe. You can add a module (I don't know about any in OCA), for having a wizard that allows you to introduce new products from the picking, adding the purchase.order.line and the stock.move/stock.move.line ones. Another option is to auto-create the PO line while validating the picking for newly created stock.move records. This may be part of the purchase_amendment module that Denis is preparing.
